Repeat prescriptions form

Patients should note that this service will be turned off on 15th January 2016 and replaced by Patient Access which will allow patients to select medications from a list rather than having to type them in each time.

In order to use Patient Access you will need a PIN number. This is the same PIN number that is currently used for appointment booking. If you don’t have a PIN number then please request one by speaking to a receptionist or send an email request to nencicb-nor.wmg@nhs.net (give your name, address & date of birth).

If you currently have a PIN number but find that you can’t order prescriptions via Patient Access then send an email to the above address and we will enable this for you.
